            Jenn Preston, Project Lively

·         Project Lively has only 2 requirements, age 60 and older and live in Douglas County.  It is a voluntary program.  Phone number for the clinic is:  785-843-0721.

·         Shannon Hodges is a new case worker, replacing Susan Ridenour who retired.

Shannon volunteers at the Oak Hill Cemetery.

·         Project Lively is a no-cost program funded by the city and county.  It was started 30 years ago as KDAG with a 3-year grant.  The program featured structured assessments for clients and worked with KU to get things started.

·         Project Lively helps individuals with unbiased information sources, help with forms and applications for service.  They get a lot of referrals from the medical community.

·         When service stops at a hospice or care facility, it is due to Adult Protective Services who are the regulators, not Project Lively.

·         Project Lively has no contract or long-term services.  There are 2 care coordinators, but there is one more position open and available.

·         Between 80-100 clients is the usual case load.  They do receive regular referrals all year long.

·         Other services at the Health Department – late hours on Monday and Wednesday for the clinic.  They also have vaccines for older adults.

The group discussed the form of LDCAA.  It is an advocacy organization.  Tamara said that it should remain independent for clear thinking.  Mary said that she would send out the mission statement and principles to the group.
