Please include the following item on the City Commission consent agenda for consideration at the October 13, 2009 meeting:

Project Description/History.  The 2008/2009 Microsurfacing Program project was awarded to Ballou Construction Co., Inc.   On April 14, 2009 the City Commission approved change order No.4 to extend the 2008 Microsurfacing Program with the proposed 2009 Microsurfacing Program project locations.  These street locations were part of the City’s 2009 pavement maintenance program that was reviewed and approved by City Commission on January 27, 2009.  The work included the patching and microsurfacing of multiple City streets.

Project Funding.  The current contract amount is $1,018,873.70 which includes both 2008 and 2009 Microsurfacing Program locations.  Due to the increase in patching performed in preparation for microsurfacing on various street locations, this project saw an overall increase of $25,800.95 over the awarded contract amount.

Action Request.  Approve Change Order No. 5 & Final to Ballou Construction Co., Inc. in the amount of $25,800.95 from Account #001-3800-578-45-10.
