Vehicle / Job Appropriateness Survey
 
The following survey is meant to help determine what type of vehicle can and should be utilized for specific job functions. This form must be submitted to the Central Maintenance Garage Superintendent along with the "Request for Vehicle Bids" form.
 
Work Assignment; what type of use? Refuse collection      
  Examples: crew truck, inspector, supervisor, staff  
 
Primary assigned driver or function? An Operator II will be the primary assigned driver
 
Size of loads (weight and dimensions): Varies by roll-off box size; approximately 1-10 tons
  (Rough estimates to help determine needed carrying capacity and size of vehicle. If this is to be a staff vehicle how many staff members will normally be carried?)
 
Types of load routinely carried: Refuse, construction/demolition material    
  Examples: Tools, equipment, paperwork, staff  
 
Frequency of load carried: Six to eight times per day, five to six days per week  
  Examples: Daily, 3-4 times/wk, 1-2 times/wk, 1-5 times/month, 1-3 times/quarter, 1-4 times/yr
 
Normal Work Routine:  
  Will this vehicle need to be driven off road? No, only at landfill  
  Purpose of off road use? Unload refuse      
  Distance of off road use? 100-200 feet      
  Frequency of off road use? Ssix to eight times per day   (week/month/year)
 
Will this vehicle need to pull a trailer? No    
  Size of trailer and load (i.e. riding mower, tractor, construction equipment, materials):
  Single axle 2,000-4,000 pound      
  Tandex axle 4,000-8,000 pound      
  Tandex axle over 8,000 pound      
 
What environmentally preferred options were considered? (summarize evaluation)  
Meets EPA emissions standards for diesel.  Hydraulic/diesel hybrid considered but not yet available for use.
