Please include the referenced item on the City Commission consent agenda for consideration at the December 30, 2008 meeting:
Project Description. The 2008 Overlay Program Phase 3 (PW0820) project is part of the annually contracted pavement maintenance program. R.D. Johnson was contracted to perform the work outlined in the Alternate B and Alternate C (Airport) portions of Phase 3. The Alternate B work included the milling and overlaying and curb and gutter replacement work on streets that include Trail Rd from Monterey Way to Folks Rd and Eldridge St from Trail Rd to Overland Dr. Curb and gutter quantities were increased to also include full curb and gutter replacement on Eldridge St from Overland Dr to W 6th St due to significant deterioration.
Project Details. The original contract was awarded for $303,685.40. Due to the increase in curb and gutter removal and replacement, this project saw an overall increase of $24,448.56 over the awarded contract amount.
Action Request. Approve Change Order No. 1 and Final to R.D. Johnson Excavating Co., Inc. in the amount of $24,448.56.
