RE:	2008 Microsurfacing Program - Change Order No.3

Please include the referenced item on the City Commission consent agenda for consideration at the December 30, 2008 meeting:

Project Description.  The 2008 Microsurfacing Program (PW0815) project is part of the annually contracted pavement maintenance program.  Ballou Construction Co., Inc. was contracted to perform the work.  The work included the patching and microsurfacing of multiple streets in the bound area generally described as between Kasold Dr and Iowa St from W 19^th St to W 31^st St.  All of the patching on the contract has been performed and approximately 15% of the microsurfacing has been completed.  The microsurfacing work has been suspended due to weather conditions and will resume once temperatures are appropriate for microsurfacing.  The projected restart will occur in early April of 2009.

Project Details.  The original contract was awarded for $548,252.70.  Due to the increase in patching performed, this project saw an overall increase of $22,000.00 over the awarded contract amount.

Action Request.  Approve Change Order No. 3 to Ballou Construction Co., Inc. in the amount of $22,000.00.
